SMS TERMS & CONDITIONS

1. SMS Consent Communication

Information (Phone Numbers) obtained as part of the SMS consent process will not be shared with third parties for marketing purposes.

2. Types of SMS Communications

If consent has been given to receive text messages from Stephen Lober Plastic Surgery, messages may be received related to the following:

  • Appointment reminders

  • Follow-up messages

  • Customer inquiries

Example: "Hello, this is a reminder of your upcoming appointment with Dr. Lober at 2325 Prince Avenue Athens, GA 30606, on 01/01/2025 at 9:00 AM. Reply STOP to opt out of SMS messaging at any time."

3. Message Frequency

Message frequency may vary depending on the type of communication. For example, up to 5 SMS messages per week may be received related to appointment reminders, follow-up messages, and customer inquiries.

BOTOX POLICY

Effective June 14th, 2024

Patients scheduling Botox injections at Stephen Lober Plastic Surgery will pay a non-refundable $75 deposit to hold their appointment. The $75 deposit will be applied to the balance of their service upon checkout. If a patient cancels within 72-hours or is a no-show, we will retain the $75 deposit. If a patient reschedules more than 72 hours prior to their appointment we will offer a one-time transfer of their deposit. If they reschedule more than once they will need to pay an additional $75.


COSMETIC CONSULTATION POLICY

Effective June 14, 2024

New Patients scheduling cosmetic consults will pay a non-refundable $75 deposit to hold their appointment. The $75 deposit will be applied to the balance of their surgery if they schedule their procedure within a week of their consultation. If a patient cancels their consultation within 72-hours or is a no-show we will retain the $75 deposit. If a patient reschedules more than 72 hours prior to their appointment we will offer a one-time transfer of their deposit. If they reschedule more than once they will need to pay an additional $75. If the patient requests multiple cosmetic consults for the same procedure they will be charged $75 for each appointment.

Established Patients will need to provide credit card information to hold their appointment, but we will not charge them unless they cancel within 72 hours or no-show for their consultation. Established patients receive one complimentary consultation per procedure, but will be charged $75 for subsequent consultations.
